Grey's Anatomy season 22 episode 18 recap: Owen and Teddy say goodbye as Meredith makes a major decision

Grey's Anatomy season 22 comes to an end with a shocking episode that sends off two beloved doctors.

Add a bridge collapse to the long list of disasters to have struck Grey Sloan! The Grey's Anatomy season 22 finale finds the hospital racing to treat those injured in a bridge collapse, which includes two of their own. It's an episode that keeps fans on the edge of their seats from start to finish and brings an end to two doctors' time on the show. So how did the finale play out and how did the writers send Owen and Teddy off?

As the episode opens, Teddy has just listened to Owen’s message as we cut to a scene of Owen in his car with a clear head injury and water quickly filling the vehicle. He comes to just as his head is about to go under and begins to race to break the window and swim to the surface. Once out of his car, he begins to take in the scene around him and search for those in need of help, as he hears a father calling out for help and his family trapped in the car.

Back at the hospital, Simone informs Jules that she and Lucas hooked up as Blue joins the conversation. While he was fired, he's hoping he can use the day to prove to Weber that he deserves to keep his job and the alert that went out did call for all available doctors. Jules catches up briefly with Winston as the pair are still on cloud nine after their night together, but that is short-lived as Teddy gives the group an update and prepares for patients to begin arriving. As they break, Wes questions Simone about going to bed early, hinting he knows something is up.

Bailey finds Catherine inside the boardroom, and Bailey asks if it bothers her that the IRB is telling her how to run her hospital. Catherine brushes it off, noting she has too much to worry about in running the hospitals.

Teddy is surprised to see Cass, who offers up her assistance and jumps into action. Cass, Simone, and Jules head out to greet an ambulance as Nick is brought in on a stretcher. Meredith and Amelia enter the ER, and he tells Meredith he loves her as she’s rushed up to the OR for emergency surgery, this after she lied about being his wife to make her way into the ER to see him.  

Elsewhere, Jo is called in to help with a pregnant patient injured in the bridge collapse. They have to think fast if they have any hopes of saving the mother and her baby, with Link suggesting an experimental treatment. Link’s plan works as Jo is able to deliver the baby as they rush the mom to the OR for surgery. 

On the scene, fire and EMS arrive to help with the family trapped in the car and the daughter’s time is running out. Owen informs the father that the only way they can save his daughter’s life is to perform an emergency amputation of her leg. As Owen performs the emergency amputation on the daughter, he encourages the brother to tell him about his baseball games to help keep him distracted as he performs the procedure in the field.

Winston comes to check on Jules as she’s struggling with seeing Nick like this, as he was one of the first to believe in them. We then cut to Cass walking along with Meredith and Amelia as she informs them they’re racing Nick up to surgery. Meredith wants to scrub in to assist, but Amelia and Cass points out how it’s not the right decision as she storms off. She tracks down Bailey and tells her that she's needed in the OR, enlisting the person she trusts most to step in to take care of Nick.

Between treating patients, Simone pulls Lucas aside and asks him if he’s heard about Nick. He’s called away, and she assures him that they’re doing all they can for Nick. We continue to see the pair dance around their secret hookup with Wes none the wiser to what went down between them the night before.

Down in the ER, Richard is shocked to find Blue working alongside Teddy. He orders Blue to grab his things and leave, reminding him he was fired but Teddy stands up to Richard and makes it clear Blue is not going anywhere. She needs hands and if anything happens on her watch, she'll take the blame.

EMTs get both of the kids out of the car and loaded away for transport to the hospital. Owen and the EMTs shift their attention to helping the father out of the car next, as he asks Owen to take his wedding ring and give it to his wife and to make sure she knows how much he loves them. Owen sits with him as the EMTs begin working to free him from the car.

At Grey Sloan, Teddy is still trying to get a hold of Owen as Richard comes out to ask what has her stressed out. She tells him about the fight they had last night, and he reassures her, telling her not to give up hope just yet. Upstairs, Jo and Link prepare for surgery, and she admits she can’t allow herself the time to think about how terrifying things are because her patient needs her. Meanwhile, it becomes clear that Link might have irritated his shoulder injury, but he keeps this from Jo in the moment.

The mother of the crash is brought in and she recognizes his keychain which he used during his triage in the field. Minutes later, Owen arrives in an ambulance with the father. Ben tries to pull Owen aside to get checked, but he wants to see this case through and heads inside to help prep the father for treatment.

Up in the OR, Amelia and Meredith sit together in the gallery to watch Bailey and Cass perform Nick's surgery. Amelia calls the way Nick and Meredith support one another really beautiful as she mentions that she can’t stop Toni from getting back with her ex. During the procedure, they notice the urine bag is empty which signals his kidney is shutting down. 

Meredith suggests they look for a clot and Bailey orders an ultrasound. Catherine comes in to check on Meredith and is shocked to see Bailey operating given she’s suspended and Meredith makes it clear she was the one who asked Bailey to step in as Bailey calls for Winston to help. Winston scrubs in to work alongside Bailey and Cass after they discovered the clot. Things are touch and go, but Winston is able to successfully clear the clot and they proceed with finishing the surgery. 

Teddy helps walk Blue through the procedure as they treat the mother from the crash, guiding him through as he pulls things off successfully. She encourages him to tell Richard why he’s different and the hospital needs him as she’s called away. Teddy walks into the OR to see Owen, who asks for her help. We can see the relief in her eyes as she assures him she’s there for whatever he needs.

Simone finds Wes hiding from Lucas. He admits that intern year sucks and that this year doesn’t feel like it belongs to him, except for when he’s with her as those parts he likes. He invites her to go out and get dinner, and she agrees as he heads off to return to work. 

Winston comes to see Jules after the surgery, and we learn Nick is stable. Jules admits that watching Meredith in the gallery with Nick made her realize that if something happened to one of them, no one would know to notify them as he admits he wants that to happen. 

Blue goes to see Richard and Catherine as he apologizes about putting the program in jeopardy. He notes that today, patients are alive because of him, but Richard does not budge and takes Blue’s badge as he heads off. 

Link finds Jo in the lounge and she admits she didn’t take the exam as she confesses that she’s not sure if she can practice medicine anymore. He assures her that whatever decision she makes, he will support her. As Jo steps away, we see Link taking several pills, hinting that he’s self-medicating. 

Bailey comes to see Ben and informs him that she was benched from surgery, but she operated anyway on Nick. Bailey admits that having a voice in hospital change requires more skills, as he jokes about her geting a master's in public health. The idea resonates with Bailey as she wants to get a seat at the table, and can’t keep doing things the way they’ve always been done.

Grey's Anatomy gives Teddy and Owen a happy ending as they exit the show

Owen and Teddy finish up surgery and we learn the entire family from the crash will be okay. Teddy tells him she’s not going to go to Paris, the only thing that makes her feel like home is Owen. This is a great opportunity, but there will be others. He tells her she is taking the job in Paris and that he’s coming with her. He’s spent his whole life chasing adventure but the best thing he’s done was their family; he’d do anything for them, and for Teddy as they’re his life. 

Nick comes to and tells Meredith as she asks him to marry her. She tells him she doesn’t want to lose him and that she doesn’t want her fear to get in their way anymore. She doesn’t want a big wedding, but she wants to marry him. 

As Lucas heads out, he finds Simone on a bench. He thanks her for opening her home to Katie and apologizes for how he treated her as he invites her out for a drink. He admits that he doesn’t regret their night together as Wes shows up and Simone leaves with him. 

Toni comes to see Amelia and informs her that her ex is coming to Seattle. Amelia admits that she understands, but to her surprise Toni reveals that she told her ex it’s over as she’s falling in love with her. The only problem is that Amelia wasn’t alone: Cass was inside. 

The episode ends with Owen and Teddy heading out following an emotional montage revisiting their highlights from over the years. The pair got a rare happy sendoff, leaving the door open for them to return again in the future while bringing a perfect end to their journeys on the show.
