Homer Simpson must donate a kidney when both of Grampa’s burst during a road trip and if he doesn’t, Grampa will die.

When the Simpsons are on their way to a nearby ghost town, their car breaks down outside the Springfield Retirement Castle. Grampa sees them and believing they remembered his birthday, climbs into the car with them.

Bur once at the ghost town, Marge is upset to learn that the town’s economy was based primarily around prostitution. Homer is bored by the tour guide, prompting the family to break off from their group and go to a saloon on the premises.

At the saloon, Grampa drinks a large number of sarsaparillas, but Homer refuses to let him go to the restroom before leaving. Grampa continually requests that Homer pull over and allow him to use the restroom, but Homer keeps refusing, leading Grampa’s kidneys to explode.

Dr. Hibbert tells Grampa that he is lucky to be alive and his best hope is for Homer to donate a kidney. Homer agrees, but is discouraged by a conversation with Carl, Lenny, and Moe, who highlight the dangers and drawbacks to kidney donation. In bed that night, Marge attempts to encourage Homer by reminding him that his father raised him.

But just before the procedure is about to begin, Homer flees the operating room. Ashamed that he has abandoned his father, Homer decides to begin a new life at sea. He winds up joining the crew of the Sea Captain’s ship, which consists entirely of other “lost souls” fleeing their past sins. Even they become disgusted by Homer’s actions and throw him overboard.
