I just love that Sam knew something was up. Dean would never bring him breakfast in bed if it wasn’t for something bad happening to him. However, he was so weak and confused that he didn’t completely question it. While I feel so bad for Sam during this storyline, this line stands out because of how much it shows about Sam’s downward spiral.

Turning on his GPS in case he tries to fly the Cuckoo’s Nest
Dean is often viewed as the dumb brother, but really, he’s smart. He’s street smart with the best survival skills. Knowing that Sam isn’t himself and is in danger, Dean does the one thing that he can to try to keep his brother safe.
This isn’t the first time we’ve seen Dean use Sam’s GPS to track him. And funnily enough, it all comes up by the end of the episode. I just love that we continue to see how street smart Dean is.
You just lost one of the best friends you ever had, your brother’s in the bell jar, and purgatory’s most wanted are surfing the sewer lines, but you know, yeah, I get it. You’re fine.
Bobby points out a lot. Not only does he bring up Sam’s situation, but he makes a point that Castiel was Dean’s best friend. When it comes to understanding Dean’s mental state, Bobby is one of the best. This is what I miss about our own Bobby.
Dean needed the wise words of Bobby in this moment. And I love that Bobby knew the way to start getting through to Dean.
Did you know a study showed that 3/4 of doctors cheat on their exams? He might not know your appendix from your vagina.
This line just always makes me laugh out loud. When I had my appendix out, I would have been so creeped out if someone had said this to me. Jody handled it like a trooper—and her face was a picture.
I am your flesh-and-blood brother, okay? I’m the only one who can legitimately kick your ass in real-time.
Finally, it’s Dean getting through to Sam. When it comes to these brotherly moments, Dean knows exactly what to say. This does end up being very similar to the lines Dean says in the Supernatural Season 8 finale when Sam is on the brink of giving up and sacrificing himself to close the Gates of Hell.
What I’ve always wondered is whether Dean means he is able to kick Sam’s ass or whether Dean is the only one allowed to. That means Hallucifer can back off.
